A temporary employee was using a torch to cut scrap. A track-hoe with a shear was working approximately 40 feet from the employee. The shear dropped a piece of scrap to the ground, striking a 6-foot long, 3 1/2" in diameter solid shaft. The shaft went airborne and hit the employee. He was hospitalized with injuries to his left shoulder, left hip, and left leg.

An employee was unloading a truck using a PIV. He backed the PIV out of the truck and noticed that some stock had fallen. He got off the PIV without setting the parking brake. The PIV rolled forward and pinched the employee's leg between the pallet and the side of the truck.
